The CLS APP3 form is an application for a Legal Aid Certificate for legal representation and investigative help. It is used to tell the LSC why you need legal aid.

How to complete CLS MEANS 1 Financial Assessment Form.
The CLS MEANS 1 form is for those not receiving Income Support or income-based Jobseeker's Allowance.
The CLS MEANS 1a form is to be completed along with a MEANS 1 if you are self employed.
The CLS MEANS 1B form is to be completed for information about an applicant’s partnership (88 & 99 Act cases).
CLS MEANS 1c is a form to be completed along side a MEANS 1 form if you are a Director or a Shareholder in a Limited Company.
The CLS MEANS 2 form is for those who receive Income Support or income-based Jobseeker's Allowance.
The L17 form is a statement of earnings: to be completed by your employer along with Means 1. You will need to ensure that your employer affixes their Company stamp on the second page in the bottom right hand corner. If they do not have a stamp, they will need to sign in the space. You will also need to have your employer confirm on their letter headed paper that they have completed the L17 form. It will help to speed up the application process if you also attach your last 3 months wage slips.
The L31 form is an application for a Legal Aid Certificate and has to be completed if you receive rental income, along with Means 1.
